Summer Team Request Form
Please fill out the form below with the players with which you'd like to play with for the 2008 Summer Program. You may request a maximum of 17 skaters, plus goaltender, that you would like to play with. You may not request more than eight players, plus goaltender, from any one Winter Team. You can fill in as few as two players on this form.
The Adult Hockey Association will make every effort to honor player requests listed on the Player Request Form, however there is no guarantee that all requests will be granted. Final team rosters and level of play will be determined by the AHA prior to the start of the season and adjustments may be made as necessary. There will be approximately 17 players to a team.
